What the Research Shows

A study published in Human Reproduction examined 656 men attending a fertility clinic and found significantly higher sperm concentration and total sperm count in men who wore cotton boxers compared to those in synthetic or tight underwear. The researchers attributed this to both temperature regulation and reduced chemical exposure.

Separate research has linked chronic low level phthalate exposure to reduced testosterone levels in men across multiple age groups. A study in Environmental Health Perspectives found an inverse relationship between urinary phthalate concentrations and testosterone levels in adult men, meaning higher chemical exposure corresponded with lower hormone levels.

None of this research points to underwear alone as the sole cause of declining testosterone. The picture is more complex than that. But underwear is one of the most consistent and overlooked sources of daily chemical and thermal exposure in an area of the body that is directly responsible for testosterone production.
